Charger Stopped Charging and Connecting to App

It’s been slightly over 3 weeks since I started using the charger and today it stopped charging, and also stopped connecting to the app. When the charger has no connection to any USB device, the 3 LEDs are blinking white. When a device is connected, they blink both white and green in a random pattern. The device is also observed to be not charging. I removed the charger from the app and tried to add it back in but the app couldn’t detect the device. Please advise. Thanks.
